Table 1 IDEA study eligibility criteria

Inclusion criteria

Exclusion criteria

SMART

• Diagnosed with type 1 or type 2 diabetes

• 18 years of age or older

• Able to provide informed consent

• African American/Black or Latine

• HbA1c level ≥ 7% measured within 365 days prior to study enrollment

• Have a smartphone compatible with the Glooko RGM application [20]

• Completed at least one appointment at the Center since the Center opened in 2020

• Fluent in English or Spanish

• Have an activated Power of Attorney

• Diagnosed with Stage V renal disease or undergoing dialysis

• Diagnosed with a severe form of cardiovascular disease

• Diagnosed with gestational diabetes only (without type 1 or type 2)

• Diagnosed with any severe form of mental disorder including schizophrenia, bipolar disorder, or severe depression

• Pregnant

• Diagnosed with Alzheimer’s disease or dementia

• Already an active user of RGM

• Actively working with a CHW for their diabetes

• Planning to travel outside of the Chicagoland area for more than three months in the year following study enrollment

• Use a continuous glucose monitor that is incompatible with the Glooko RGM application

Participant Focus Group Discussions

• Meet all eligibility for SMART component of study

• Completed at least 3 months of their assigned SMART study condition

None

Center Staff Interviews

• Employed by the Center during the study period

• 18 years of age or older

• Able to provide informed consent

• Fluent in English or Spanish
